[Girin Plastic Surgery] - Cleft Lip and Palate Nose Reconstruction Surgery

Today, I would like to discuss cleft lip and palate as well as nose reconstruction surgery.  


Cleft lip and palate, one of the most common congenital deformities of the face, refers to a condition in which the lips or palate are split into one or two branches due to the failure of the tissues that make up the lips and palate to properly attach between the 4th and 7th weeks of pregnancy. If only one side is affected, it is known as a unilateral cleft lip and palate. If both sides are affected, it is referred to as a bilateral cleft lip and palate. In both cases, primary corrective surgery is typically performed within a few months after birth.

However, even after primary correction, secondary deformation may occur along with bone growth during the growth period. During the growth period, bone growth should develop evenly, but in most patients with cleft lip and palate, the growth of nasal cartilage and palate is abnormally slow and underdeveloped compared to other parts, resulting in deformation of the nose shape, philtrum around the nose, and asymmetry of the lips.



Therefore, even after becoming an adult, there may be a need for second or third cleft lip and palate nasal reconstruction surgeries to address lip and philtrum scars, as well as nasal deformities.

The nose, being at the center of the face, plays a crucial role in one's overall appearance, and even minor deformities or asymmetry can significantly impact one's facial impression. Therefore, any decision regarding surgery should be approached with careful consideration. In certain cases, structural reconstruction may become a necessity. To establish a comprehensive surgical plan, a thorough examination of the interior of the nose is imperative. This ensures that the chosen approach is tailored to the individual's unique needs and circumstances.




Cartilage asymmetry, such as a deviated nasal septum, a drooping nose tip, or nostril irregularities, can lead to nasal distortion. This condition involves structural issues that make it more complex to address than a typical nose. Consequently, correcting it completely in a single surgery can be challenging. It necessitates a meticulous approach that combines structural design and functional reconstruction of the nose, making it more intricate than a standard rhinoplasty procedure. While achieving perfect symmetry can be demanding, a crucial step towards success is undergoing a CT scan for a precise diagnosis and choosing the most suitable implant for the surgical procedure.

The following shows cleft lip and palate nasal reconstruction surgery performed at our clinic.

Case1: where the tip of the nose sinks after cleft lip and palate surgery.

 

This patient does not exhibit severe asymmetry in her nasal structure. Overall, her nose presents a good balance, with no noticeable deviation in its axis. However, there is a concern regarding the underdevelopment of cartilage at the nasal tip, giving the appearance of a smaller nasal bone and a somewhat pressed-in appearance on the front and sides. Additionally, the tip of her nose displays some bulbosity, which suggests that our focus should primarily be on addressing the nasal tip.

Given that her skin lacks elasticity, there are inherent limitations in raising the nasal tip. Therefore, to achieve the desired outcome, we are considering the use of ear cartilage, as the nasal septum alone may not provide sufficient support for the necessary adjustments. Furthermore, we believe that reducing the alar base using specialized techniques can contribute significantly to enhancing the overall beauty and symmetry of her nose.

Results

Photos below show a remarkable transformation between before and after the surgery from multiple angles.

From the frontal and side views, it is evident that her depressed nose has been improved. Her nasal bridge and tip have been artistically elevated, and by reducing the wide nasal, we were able to achieve a much more refined appearance than before.
